We understand that surrendering a dog is a difficult and emotional decision to make. Rest assured that SCMR will provide your dog with the care and love they deserve. Our team is committed to finding every dog a loving forever home.

Owner Surrender Form

Please be advised before proceeding with filling out the following information, that SCMR requires all necessary vetting expenses be paid for by the owner, including but not limited to: bring up to date all shots, spay/neuter, dental, heartworm test.

If you prefer mailing the application instead of using the online submission method, please print the PDF version and mail it to SCMR at: SCMR; P.O. Box 2005; Chattanooga, TN 37409.

It is important to be sure the entire application is filled out correctly and completely. If any questions are not completed, the application with not be processed.

Having read this notice, I acknowledge I understand these policies.

Provide contact information of Veterinarian below. SCMR will also require you to contact your Veterinarian and let them know we will be calling to verify the provided information, giving SCMR permission to get the needed information.

This surrender request may be executed by completing all of the information asked for, and then sending to Southern Comfort Maltese Rescue.  By sending this form in, it does not mean that Southern Comfort Maltese Rescue will take the dog, it only means that our rescue will consider the request sent.

 

The person(s) making this request is submitting the form and information voluntarily, and will not hold SCMR responsible or liable for any action taken.
